Support Coordination

Your NDIS plan can be tricky to navigate, with complex rules and many paths to choose to achieve your goals.

Support coordination is a service for participants who have ‘Coordination of
Supports’ in their NDIS plan. A support coordinator will draw on experience
and community connections to make sure the right people and supports are
in place for you to meet your goals.

A Support Coordinator will support you to:

  • Understand and implement the funded supports in your plan.
  • Help you exercise your choice and control
  • Find the right people to support you.
    Help you work out how to do some of the things you want to do in the
    community.
  • Help you reach agreements with providers and ensure service
    agreements are in place.
  • Assist in monitoring your NDIS budget and expenditure
  • Support you to work towards your goals and live your life the way you
    want it to be.
